   Proposal Description:
   Abstract:  Adverse drug events (ADE) are one of the main preventable factors leading to increase in health care related costs in the elderly. Potentially inappropriate medication use due to complex comorbidity profiles could lead to an increase risk for ADE and death in this population. The risk could be even higher in patients with cognitive impairment.  We propose a retrospective analysis using the data collected in the ongoing National Alzheimer???s Coordinating Center (NACC) cohort study. These rich data would allow for longitudinal evaluation of medications use while also capturing important information on demographic characteristics, medical history, and functional and cognitive status.    The specific objectives are:  Specific Aim 1: To evaluate prevalence of potentially inappropriate prescribing in the NACC cohort by levels of cognitive function.   Potentially inappropriate prescribing will be measured by:   i. The 2012 American Geriatrics Society Beers Criteria for Potentially Inappropriate Medication Use in Older Adults  ii. The Health Effectiveness Data and Information Set (HEDIS) measures of prescribing quality (i.e. use of drugs to avoid in the elderly and drug-disease  drug-drug interactions in the elderly).  Specific Aim 2: To identify patient characteristics (i.e. demographics, comorbidities, and functional status) associated with potentially inappropriate prescribing in the NACC cohort and determine if they differ for different levels of cognitive function.  Specific Aim 3: To measure the prevalence of anticholinergic use in participants treated with approved FDA medication for Alzheimer???s disease in the NACC cohort and to compare rates of anticholinergic use before and after the initiation of Alzheimer???s disease treatment (i.e. examine whether anticholinergic drugs were discontinued upon Alzheimer???s disease treatment inception).
